PAYMENT OPTIONS:

The Office Coordinator can assist you with payment options and treatment planning.  You can pay for office visits and any treatments by cash, check, credit or debit card at the time of service. We also accept most major dental insurances.  Financing can also be obtained through CareCredit and Capital One. To discuss your payment options or insurance coverage, please call 801.489.9494.

  

CANCELLATION POLICY:

So that we may better serve you and other patients, we ask that notification of appointment cancellation be given to our office no less than 24 hours before your scheduled appointment.
